ABSTRACT

IgA nephropathy is the most common primary glomerular disease in China. Its clinical manifestations are mainly proteinuria, hematuria, hypertension, edema, hyperuricemia, etc. Most patients have hidden onset. 30%-40% of patients develop into end stage renal disease 10-20 years after diagnosis and rely on dialysis or kidney transplantation to maintain their lives, which is extremely harmful. Proteinuria is a common clinical manifestation of this disease, and most patients have small-to-moderate amounts of proteinuria, while 10%-24% of patients have large amounts of proteinuria. Proteinuria is the main risk factor affecting the progression of renal function in IgA nephropathy. Podocytes are the terminal part of the glomerular filtration barrier, and various factors can affect the fusion and detachment of podocyte processes that occur after podocyte injury. They are common histological lesions in IgA nephropathy and are key factors leading to proteinuria and the continuous progression of the disease. At present, Western medicine lacks targeted treatment for podocyte injury, with limited intervention methods. Drugs such as glucocorticoids are often used for treatment, and there are many adverse reactions. Based on the physiological function of podocytes, pathological and physiological changes after injury, and histological morphology of this disease, it is believed that it is closely related to traditional Chinese medicine's "Xuanfu Theory" "Kidney Collateral Syndrome" "Collateral Disease Theory", and "Dry Blood Theory". More and more studies have shown that traditional Chinese medicine, which has the characteristics of multiple links, pathways, and targets, has a significant therapeutic effect on podocyte injury in IgA nephropathy. It can protect podocytes and reduce proteinuria and has good application and research prospects. This article systematically summarizes the mechanism and morphological changes of podocyte injury in IgA nephropathy, the understanding of podocyte injury in traditional Chinese medicine theory, and the research progress in traditional Chinese medicine treatment of podocyte injury in IgA nephropathy, so as to provide a reference for further research and application of traditional Chinese medicine intervention in podocyte injury in IgA nephropathy.

ABSTRACT

The modernization of traditional Chinese medicine (TCM) theories is not only a prerequisite for the modernization of TCM but also a way to deal with the external environment and the difficulties of its development. The modernization of TCM is based on the premise of maintaining its own characteristics and advantages, and along with the progress of modern science and technology, the entire TCM has undergone new changes and improvements from theory to practice, and has become a scientific system with modern scientific level that adapts to the needs of the times. In the context of the unprecedented profound changes in contemporary TCM academics, this paper puts forward the definition of modernization of TCM theory and argues that the modernized theoretical system of TCM should maintain originality to build the foundation of TCM inheritance, clarify the frontier of the discipline and establish the conceptual system, form an open network to be compatible with multi-disciplinary interaction, incorporate the falsification and establish self-renewal system, creatively transform and innovatively develop, and modernize the interpretation and update the discourse system.

ABSTRACT

In order to judge the future development trend of science and technology, plan ahead and lay out the frontier technology fields and directions, China Association of Chinese Medicine(CACM) has launched consultation projects for collecting "major scienti-fic issues and engineering technology difficulties in traditional Chinese medicine(TCM)" for the industry for three consecutive years since 2019. Up to now, 18 projects have been selected as major issues for research, and some experience and achievements have been made. These projects have been applied in important scientific and technological work such as scientific and technological planning and deployment at all levels of national, local, and scientific research institutions, the selection and cultivation of major national scientific and technological projects, and the construction of innovation bases, giving full play to the role of the think tank advisory committee of CACM. This study reviewed the selection of major issues for the first time, systematically combed its application in the national layout of science and technology, and put forward the existing problems and improvement suggestions, aiming to provide new ideas for further improving the selection of major issues and research direction, providing a theoretical basis and decision support for the national scientific and technological layout in the field of TCM, and promoting scientific and technological innovation to facilitate the high quality development of TCM.

ABSTRACT

The tight relationships between microbiome and traditional Chinese medicine(TCM)have been widely recognized. New technologies, results, and theories are emerging in the field of microbiomics in recent years with the advances in high-throughput sequencing and multi-omics technologies. Based on the previous research, the present study has proposed the concept of TCM microbiomics(TCMM), which is an interdisciplinary subject aiming at elucidating the functions and applications of microbiome in the areas of herb resources, herb processing, herb storage, and clinical effects by using modern technology of biology, ecology, and informatics. This subject essentially contains the structures, functions, interactions, molecular mechanisms, and application strategies of the microbiome associated with the quality, safety, and efficacy of TCM. Firstly, the development of the TCMM concept was summarized, with the profound understanding of TCMM on the complexity and entirety of microbiome being emphasized. Then, the research contents and applications of TCMM in promoting the sustainable development of herb resources, improving the standardization and diversification of herb fermentation, strengthening the safety of herb storage, and resolving the scientific connotation of theories and clinical efficacy of TCM are reviewed. Finally, the research strategies and methods of TCM microbiomics were elaborated from basic research, application research, and system research. TCMM is expected to promote the integrative development of TCM with frontier science and technology, thereby expanding the depth and scope of TCM study and facilitating TCM modernization.

ABSTRACT

ObjectiveTo investigate whether the whole intestinal microbiota transplantation in Alzheimer's disease (AD) model mice has more significant effects on ileum intestinal microenvironment in normal mice under the guidance of the theory of traditional Chinese medicine that "interior-exterior relationship exists between the heart and small intestine". MethodsThe whole intestinal microbiota of fourteen 6-month-old specific pathogen free male APP/PS1 double-transgenic AD model mice was transplanted into the gut of six normal C57BL/6J mice of the same age and background treated with mixed antibiotics for 14 days. Then, after 14 days of normal rearing, the mice were sacrificed. Next, the pathological changes in the ileum and colon were observed, and the composition and diversity of the ileal and colonic microbiota was analyzed by sequencing. ResultsAfter the whole intestinal microbiota of AD mice was transplanted into normal mice, pathological analysis showed that only the ileum tissue had mucosal damage and crypt gland epithelial cell degeneration, necrosis, and shedding. Moreover, the microbiota analysis found that only the number of genera (P<0.01), Chao1 index (P<0.01) and Simpson index of ileal microbiota in normal mice decreased (P<0.01), and the composition of intestinal microbiota was quite similar to that of AD model mice. ConclusionUnder the effect of whole gut microbiota transplantation in AD mice, the diversity and composition of ileal microbiota change more than that of colonic microbiota in normal mice, and at the same time, it results in pathological damage to the ileal mucosa, indicating that the ileal microenvironment may be more closely related to the occurrence and development of AD, which is highly consistent with the traditional Chinese medicine theory of "interior-exterior relationship between heart and small intestine".

BACKGROUND: Although ancient physicians had rich discussions on rheumatoid arthritis, few have discussed based on the theory of collaterals diseases. OBJECTIVE: To summarize the drug treatment classification, etiology analysis, and “collateral-interest accumulation.” METHODS: CNKI, El, SCI, and ISTP databases were searched for articles published after 1900. The keywords were “The bone erosion of rheumatoid arthritis, TCM collaterals theory.” Data from more than 10 domestic and foreign articles were selected to analyze the drug treatment classification, etiology analysis, and “collateral-interest accumulation.” RESULTS AND CONCLUSION: (1) One of the causes of rheumatoid arthritis bone disease is the collaterals disease of traditional Chinese medicine. The development of intractable diseases should follow the law of the development of diseases from the meridians to the collaterals, from the gas to the blood, and from the functional lesions to the organic diseases. (2) The pathogenesis of rheumatoid arthritis lies in the accumulation of collaterals, which causes joint swelling, joint deformity and damage. (3) The treatment of “collaterals for the use of circulation” for rheumatoid arthritis is to keep the collaterals, organs and blood-gas in fluent, thus ensuring the fluent circulation to alleviate the symptoms of rheumatoid arthritis.

ABSTRACT

At present, the 2019-novel coronavirus (2019-nCoV) is rampant all over the country, and controlling the spread of its epidemic has become a top priority. It is very difficult to control the epidemic spreading because there is no specific drug to fight against it. Therefore, it is urgent task for us to develop a specific drug as soon as possible, and attracting worldwide attention. The studies on anti-2019-nCoV drugs were generally carried out in three modes, as vaccine, chemical drugs and traditional Chinese medicine (TCM). Because the developing on vaccines and chemicals takes too long, it is hard to solve the urgent problem. Moreover, the RNA of coronavirus has been recombined so quickly that the vaccines and drugs studied with great efforts not only may miss the epidemic time, but also cannot be used in the next generation of new coronavirus treatment, as a result that the work of epidemic prevention is formidable. However, the prescriptions from traditional Chinese medicine theory can cure the epidemic disease, but how to develop it into a specific drug with clear chemical composition and clear pharmacologic mechanism, is also an urgent problem to be solved. In this paper, the new coronavirus and its drug development are reviewed, while the advantages and disadvantages are analyzed, and the drug dilemma research phenomenon is clarified by using the supramolecular “qi chromatography” theory. It is suggested that the strategy of anti-coronavirus drug developing should be put forward based on the impact of “qi chromatography” of biological supramolecular “imprinting template”, in order to develop new drugs against 2019-nCoV to control the spread of the epidemic as quick as we can, and establish a new research and development model of anti-coronavirus drug on integration of traditional Chinese and Western medicine, turning passive into active.

ABSTRACT

Diabetes is one of the most common chronic non-communicable diseases. The incidence rate is on the increase year by year. As the disease progresses, it can cause damages to many tissues and organs, such as eyes, kidneys, nerves, heart and blood vessels, which seriously endangers human health. Traditional Chinese medicine (TCM) has outstanding clinical advantages in the prevention and treatment of diabetes, with distinctive characteristics of diagnosis and treatment. TCM has a long history of understanding diabetes. In recent years, researches on etiology, pathogenesis and treatment have gradually deepened and made certain progress. As early as in the Huangdi Neijing, there is a record of "If people’s lung, spleen viscera (stomach), liver, kidney are weak, they can easily get diabetes". This paper collected and summarized ancient literatures relating to diabetes. According to the basic construction principle of "integration-authenticity-innovation" in the theoretical framework of TCM,the causes of the disease include liver weakness and failure in dredging and regulation,emotional disorder, endogenous heat and dryness, heart weakness, heart Yin depletion, endogenous dryness and heat, spleen deficiency and failure in transportation and transformation, lung injury caused by dryness and heat, dysfunction of body fluid distribution, kidney Yin deficiency, astringent weakness, insufficient Qi transformation for maintaining body fluid metabolism balance. In general, Yin deficiency is the root cause, and dryness and heat are the appearance. A framework for the treatment of diabetes was built with five organs as spatial dimension. This paper summarizes the etiology and pathogenesis of diabetes caused by viscera deficiency, with the treatment of diabetes with the five internal organs as the basic framework. The main methods of treating diabetes are relieving Qi stagnancy in liver, regulating the mood,clearing the heart fire, Yin-enriching and dryness-moistening, reinforcing spleen Yang, invigorating spleen and removing dampness, clearing lung and moistening dryness, nourishing Yin, kidney Yin, warming kidney and promoting Yang. A clear and hierarchical theoretical framework was built as a bridge between the theory of TCM and clinical practice of TCM, in the expectation to provide new ideas for the study of ancient TCM literatures and the treatment of diabetes mellitus with TCM, and contribute to the inheritance and development of TCM theory.

ABSTRACT

The scientific interpretation of the connotation of traditional Chinese medicine (TCM) theory is an important part of the development of TCM. Combined ancient classic theories with modern science and technology is a new path for the innovative development of TCM theory. Based on this, taking Zhishi Xiebai Guizhitang as an example, the molecular mining technology of integrative pharmacology-based research platform of TCM V2.0 (TCMIP V2.0) was used to analyze the mechanism of Zhishi Xiebai Guizhitang in preventing and treating gastropathic stomachache. A total of 220 chemical components in Zhishi Xiebai Guizhitang were obtained, and 674 targets were involved, of which 12 core targets directly affected angina pectoris and gastroesophageal reflux disease, including insulin (INS), glyceraldehyde-3-phosphate dehydrogenase (GAPDH), RAC-α serine/threonine-protein kinase 1 (AKT1), tumour protein p53 (TP53), albumin (Alb), mitogen-activated protein kinase 3 (MAPK3), interleukin-6 (IL-6), etc. And tumor necrosis factor (TNF) signaling pathway, phosphatidylinositol 3-kinase/protein kinase B (PI3K/Akt) signaling pathway, IL-17 signaling pathway, hypoxia-inducible factor-1 (HIF-1) signaling pathway and other 121 pathways involved in these related diseases. All of these are the biological basis of Weiluo Tongxin theory. Combing the classical theories of TCM combined with the exploring the molecular mechanism of representative prescriptions can provide a demonstration and reference for the scientific connotation research of TCM theory.

ABSTRACT

The clinical value is the core factor for determining the products of Chinese patent medicines on the market. It is clear that defining the clinical positioning of Chinese patent medicines and maximizing their clinical efficacy and value are of great practical significance for each Chinese patent medicine. Precise clinical positioning not only requires systematic analysis and summarization of existing literature and clinical practice data, but also requires verification and proof from detailed experimental data, thus to provide scientific support for basic research. In this research, a three-dimensional integration strategy of " traditional Chinese medicine (TCM) theory-clinical practice-basic research" was proposed to play a vital role in clinical positioning and evaluation of Chinese patent medicines. Yixinshu capsule was taken as an example to discuss clinical positioning of Chinese patent medicines under the guidance of TCM theory. Based on the interpretation of the evolution of the pathogenesis of coronary heart disease (thoracic obstruction), combined with the core efficacy of the drug composition, the effect of Yixinshu capsule in benefiting Zongqi, calming midnd, improving blood circulation and eliminating blood stasis was identified, which was suitable for treatment for the recurrent episodes of angina pectoris, and emergency of symptoms of chronic heart failure. At the level of TCM theory, the clinical positioning of Yixinshu capsule was further defined, and the key points of in-depth research were proposed to provide reference for the mechanism research and clinical application of Yixinshu capsule. Based on the prescription and its score functions, precise analysis of clinical positioning of large varieties of TCM under the guidance of TCM theory, on the one hand will give full play to the value of TCM original knowledge and guide the basic research of Chinese patent medicines, on the other hand the research results will enrich, benefit and sublimate the TCM theory, and finally provide a critical paradigmatic for the clinical positioning of large varieties of TCM under the guidance of TCM theory.

ABSTRACT

Severe community acquired pneumonia (SCAP) has raised substantial concern due to high morbidity and mortality. SCAP is usually associated with multiple organ dysfunction syndrome (MODS) and sepsis. It is suggested that organ function and coagulation/fibrinolysis improvement, organ injury reduction and organ repair facilitation are highly required besides microbiology detection and optimization of antibiotics strategy on treatment of SCAP. Neutralization antibody, steroid, immunmodulatory therapy, and Chinese medicine etc. contribute to organ function improvement from different perspective. The unique profile of multiple target and personalized therapy of Chinese medicine may become a promising new option for SCAP and MODS. The combination of Western and Chinese Medicine shed new light on SCAP treatment.

Chinese materia medica (CMM) health products play an important position in the field of health products at home and abroad because of its long history of health, unique diet theory, and wealth resources of medicinal herbs. To inherit the theory of traditional Chinese medicine (TCM) and develop the health products with TCM characteristics, this paper has summarized the development status and common problems of CMM health products in recent years, and put forward the design idea based on the theory and advantage of TCM for designing the development model of CMM health products more scientifically and reasonably, which has followed the theory of dialectics of eight aspects, five elements, physique theory, and so on. An effort is made to create health products with TCM characteristics and advantages in order to adapt to the health needs of different people, which would make significant contribution to the people's health and industrial development.

Chronic kidney disease (CKD) is a chronic renal structure change and dysfunction caused by a variety of causes.The incidence and prevalence of CKD have been on the rise,becoming a major global health problem.An increasing number of studies had shown that one of the important indicators of CKD progression was the disorder of gut microbiome and elevated levels of sernm endotoxin.Modern physicians of traditional Chinese medicine (TCM) believed that the main pathogenesis of CKD was spleen and kidney deficiency.And the therapeutic regimen was invigorating spleen and nourishing the kidney.Spleen is in the middle burner.It has the function of transportation and resistance to evil,which was extremely similar to the function of gut microbiome in metabolism and immunity.The latter might be one of the biological bases of physiological function of spleen.This comprehensive review presented the modern theoretical basis of spleen-based TCM treatment methods for CKD from the insight of modern gut microbiome and metabolism.Firstly,gut microbiome and its metabolism were associated with the spleen in TCM theory.Gut microbiome affected individual metabolism and immune function,which could provide a modern biological basis for explaining the physiological function of the spleen.Secondly,gut microbiome and its metabolism were correlated with CKD:intestinal flora disturbance occurred in CKD patients,producing enterogenous toxins as their metabolites,which become the important factors affecting prognosis.Thirdly,as for its origin,development and current status based on the spleen theory,CKD was one of various consumptive diseases.Spleen insufficiency was one of their critical pathogenic mechanisms.Therefore,tonifying the spleen should be a treatment method to maintain throughout the entire treatment.On the basis of our previous clinical studies and the hypothesis of gut microbiome and metabolism,we proposed further studies which were also based on system biology and its technology.

Diabetic kidney disease (DKD) is a serious long-term complication and major death cause of patients who have suffered from diabetes mellitus (DM),which is a serious threat to the health of human being.Professor Zhao Zongjiang first came up with the of DKD "consumptive kidney disease" scientific theory,and led the scientific research team for DKD research.Our team had achieved some results,which were mainly focused on the animal experiments and cell culture experiment.And the research fields covered pharmacodynamics,oxidative stress levels and signal transduction pathways.Focusing on the reduction of extracellular matrix in the glomerular mesangial area and tubulointerstitial deposition,inhibition of podocyte injury,inhibition of podocyte apoptosis,inhibition and reversal of podocyte transdifferentiation,inhibition of renal tubular epithelial cell transdifferentiation and other scientific issues,this paper systematically explored the biological mechanism of DKD "consumptive kidney disease" scientific theory.This paper explained how "consumptive kidney disease" scientific theory was proposed and summarized related research results of our research team and developed traditional Chinese medicine (TCM) theory by supplying the TCM "consumptive organ disease" theory.

ABSTRACT

Objective This study aimed to understand the continuous physiological parameters during sleep under the guidance of Chinese medical theory and clinical practice and to establish the method to diagnose the physical and mental status by analyzing nocturnal sleep data.Methods More than 2000 subjects were recruited in the nocturnal sleep examination using the micro-movement sensitive mattress sleep monitoring system.Based on the analysis of the sleep indices and nocturnal physiological parameters combined with clinical data,a hypothesis was put forward that sleep cycle could reflect the circulation status of qi and blood,as well as some preliminary exploration on revealing the status of qi,blood,yin and yang.Results Sleep structure could reflect the status of qi and blood circulation in different meridians according to the traditional Chinese time unit.Sleep structure and sleep cycle could show corresponding changes if there is morbidity in some meridian,zang-viscera and fu-viscera.Heart rate,respiration rate and other physiological parameters could reflect the alternative predominance of yin and yang.Conclusion Interpreting the physiological parameters during sleep could be a supplementary diagnosis method for Chinese medicine syndrome differentiation.

@# Objective To develop a scale of social adaptation degrees of special service personnel (referred to as scale) applying Traditional Chinese Medicine and psychological theory and to assess its reliability and validity. Methods By literature review, job analysis, inventory survey, topics interview, this scale was determined in dimensions, items and items pool based on theoretical model. Results After specialist important analysis and combining the different results of statistical analysis, including Cronbach's α coefficient, resolution factor, correlation analysis and confirmatory factor analysis, 6 dimensions, 14 factors and 40 items were finally kept in the scale. The dimensions were psychological characteristics, physical condition, intelligence level, interpersonal relationships, environmental factors and moral character. The factors were self-control, self-esteem character bias, self-confidence, physiological function, learning intelligence, creating intelligence, leadership intelligence, gregariousness, cooperative, confidence feeling, natural environment, social environment and moral character. Conclusion This scale was made on the basis of general international principle, it had good reliability and validity meeting measurement standard of traditional Chinese medicine and psychological theory, and it is an effective and practical evaluation tool of social adaptation degrees of special service personnel.

ABSTRACT

The Chinese character of "phlegm" went though a process of evolution from "涕"、"沫"、"汁"、"饮"、"涎",to "淡",and finally "痰".Studies show that there is no"痰" in Inner Canon of Huangdi,but "淡"、"惔"、"澹" which are not related with phlegm;while mucus coughed up from the body is referred to as "涕"、"沫"、"汁"、"涎",and fluid retention as "饮"、"沫"、"汁".During Han and Jin Dynasty,it was called "涕"、"涎沫"、"淡饮".The character "痰" first appeared in Sui Dynasty and was separated from "饮".Since Tang Dynasty,the definition of phlegm has been established as the mucus excreted from the body or accumulated within the body,diseases and syndrome manifestations caused by fluid retention and the mode of thinking on complicated diseases and syndrome manifestations.

Intercourse between heart and kidney and harmonization of fire and water are important concepts in TCM.At present,discussions about them are not clear,but confused.This paper concludes the concepts and meanings about intercourse between heart and kidney and harmonization of water and fire,based on opinions of ancient theories of yin and yang,and five elements.It also expounds the differences and relations about harmonization of yin and yang,harmonization of water and fire,and intercourse between heart and kidney,and puts forward that the concept of intercourse between heart and kidney should be divided into aspect in two parts which are in broad sense and in narrow sense.In broad sense,it indicates kidney qi and heart qi interacts in all aspects;in narrow sense,it indicates the intercourse between heart fire and kidney water.The pathway,the time and other meanings about intercourse between heart and kidney are also reviewed in this article.

ABSTRACT

As a medical terminology,"insidious yang"is commonly seen in ancient medical literatures of TCM,but the concept,reason and clinical significance of this term has not been illustrated comprehensively.By reviewing the ancient medical literatures and the application in modern clinic,this paper brings to light that"insidious yang"has the syndromes of interior heat and fire stagnation;and the reasons include excess yang in the interior keeping yin externally,insidious pathogenic factor in the interior transforming into heat and yang qi blocked by either visible or invisible pathogenic factors.In general,interior heat can manifest as insidious yang,insidious heat and insidious fire,according to the degree of the illness.Diagnosis must be based on the 4 diagnostic methods,and the area,the season and the condition of patient should be taken into account.Treatment of insidious yang is to clear and purge against the cause of disease.At present,the syndromes of insidious yang is commonly seen.Better therapeutic effect can be achieved when the clearing and purging herbs are applied.
